This is a regional position with duties supporting Norwich, Plainfield and Windham. Hours are day time with some flexability.

This individual will provide regional support to Hartford HealthCare s East Region Emergency Department s ( Norwich, Windham, Plainfield) to enhance patient throughput, experience and outcomes.
The Nurse Navigator will serve as a middle man between the patient and the clinical care staff. The NN will help navigate the patients through the treatment process by connecting them to the necessary resources and information they need to make informed decisions. The NN assists the patient from initial diagnosis to end of necessary treatment.
The NN will help navigate the patient through all things post hospital stay by connecting them to the necessary resources and information they need to make decisions on everything from lab results, any needed prescriptions/calling the pharmacy to change/initiate RX s and even helping to find resources to pay for RX s.
The NN will utilize data collected to make informed decisions to drive outcomes.
Qualifications
BSN required; Master's degree in nursing or health/business field preferred
At least three (3) years of experience in a clinical area (such as Emergency Department) or program management. Three (3) years of ED experience preferred.
Licensure, Certification, Registration:
Current State of CT RN license
Language Skills:
Highly effective verbal and written and communication skills

Estimated Salary: $20 to $28 per hour based on qualifications.
